Acrobat or Adobe Acrobat Reader™ installed in your browser software to view,
complete, print, email or save to your computer. You can
download or upgrade a free copy of this software by clicking on the icon at right.

The application is not compatible with Mac "Preview" software. Mac
users will need to upgrade or download Adobe Acrobat Reader™.

Apple users may overcome certain compatibility issues by
switching to the Internet Explorer web browser before
accessing the application.

1. Fill in the Application ONLINE.
2. Print the completed form. (Keep as a backup copy)
3. Save the completed form. (Keep as a backup copy)
4. Click the SUBMIT button at the end of the application.
5. Your email client will create a "new email" to Emmaus.
6. The application is a PDF attachment to your new email.
7. Send the email.

If you experience difficulty sending the online application,
consider the following options.
     (a) Fax the printed copy to (252) 822-0122.
     (b) Create a new email to Emmaus and attach the saved copy.
     (c) Scan the printed copy and email.
     (d) Mail the printed copy.
     (e) Call Emmaus at (252) 393-3556 for assistance.

Each individual applicant is to complete an ONLINE Application. If your Application information is incomplete please SUBMIT now and email, fax, call or surface mail any incomplete information to Emmaus when available.
